Epalrestat inhibits aldose reductase, an enzyme involved in the formation of sorbitol in nerve cells. High levels of sorbitol are associated with nerve damage in diabetes. Pregabalin acts on calcium channels in the nervous system, reducing pain signals and nerve excitability.
